WDP has been providing drug and alcohol services in the London Borough of Barnet for eight years. We work innovatively with communities and individuals affected by substance misuse to deliver recovery focused interventions irrespective of circumstance. We are now seeking to recruit a dynamic and passionate GP Shared Care Practitioner to contribute to the ongoing success of the service. 

This exciting role will offer the successful candidate the chance to make a lasting difference to the lives of our service users and be part of an integrated service delivery model within Barnet.

The post holder will have substantial experience of working within a Shared Care setting and in a recovery focused way in order to improve health inequalities within the Borough.

The successful applicant will:

  • Have a caseload of service users and proactively engage those not currently engaging with the service
  • Agree care plans for service users presenting with drug and alcohol use
  • Work with service users to achieve their care plan objectives; this will include supporting and delivering interventions
  • Support colleagues by providing training and advice regarding the management of drug and alcohol users and their needs
  • Work in partnership with clinical and psychosocial colleagues and external partners
  • Hold clinics to assess the needs of service users
